By signing this form, I agree that the following statements are true:

• The project has been tested by a representative of the above-mentioned company, and all issues (bugs) have been provided to Human Pixel either in a document or via Zoho Projects
• That there are no further issues (bugs) to be raised, and this is a full and final list
• Human Pixel is approved to proceed to fix issues (bugs) and move to the next stage of the project
• A final invoice will be issued on completion of this form
• Payment of this invoice will be made prior to bugs being solved and Go live
